<p>As her son Richard lies on the floor, Holly Carter plays patty-cake with his feet.<br />When the 4-year-old swiftly crawls across the floor, she&#8217;s right with him. He leaps up and darts across the room; Carter is by his side. Although it looks like she&#8217;s performing some sort of maniacal mirroring marathon, playing this intensely is not all for fun and games. Richard is autistic, and doesn&#8217;t speak.<br />Their activity is part of a three-year research study of the Play and Language for Autistic Youngsters (known as the P.L.A.Y. Project) home consulting model, a parent-training program that addresses the need for intense early intervention for young children with autism. Parents commit to playing with their child 20-25 hours a week with consulting from a play therapist. The goal of the project, supported with a $1.85 million grant from the National Institute of Mental Health, is to see whether this type of intensive play can help children with autism come out of their shells.<br />In a nation where about one in 150 children are diagnosed with autism, such a program provides a glimmer of hope for parents who wish something could become different. Because it&#8217;s the fastest growing disability in the United States, too few people are trained to handle the intensive intervention recommended by the National Academy of Sciences. The P.L.A.Y. Project, the largest study of its kind, is designed to address the shortage by using a &#8220;train the trainer&#8221; approach, which is helping to rapidly spread the program around Michigan, the nation and the world.<br />While other therapies can cost as much as $60,000 a year, play therapy at $4,000 is much cheaper. Only about 150 children participate in the actual research study, but about 400 Michigan children receive play therapy as part of 15 play projects across the state, according to Dr. Richard Solomon, an Ann Arbor-based pediatrician and developmental and behavioral specialist who founded the P.L.A.Y. Project. Nationally, about 3,000 children are in therapy, and the technique already is being used in Australia and England.<br />Solomon says he hopes to see tens of thousands of children helped by this fast-spreading form of therapy.<br />&#8220;We&#8217;re training the parents to be sensitive and responsive to their child&#8217;s interest and this has been shown to be a very powerful way of helping children, especially with autism, to connect to their caregivers,&#8221; Solomon says. &#8220;Once the child is connected, their development gets engaged, and that&#8217;s what leads to improvement.&#8221;<br />Carter understands exactly what he means. Since the Howell resident began the therapy, she notes, her son has come a long way.<br />&#8220;When we first started, he wouldn&#8217;t notice anyone else in the room. He wouldn&#8217;t even look at anyone,&#8221; Carter says. &#8220;Now, he laughs out loud when he&#8217;s tickled, grabs a pretzel from a visitor&#8217;s hand and chomps into it, laughs and runs away. He wanted to feel the snow settling on the balcony separating his playroom from the outdoors, and climbs up the side of the stairs trying to get around a baby gate so he could go upstairs to watch his favorite movie.&#8221;<br />Adam Brode, a P.L.A.Y. Project home consultant and speech language pathologist, has helped with that. Not only has he modeled how to play with Richard, he&#8217;s consulted with Carter about how to go about playing with Richard a minimum of 20-25 hours a week. She says it was difficult for her to catch on to how to play with her son, finding herself making it a mechanical exercise instead of an intuitive and natural experience at first.<br />&#8220;We&#8217;re making sure nothing stands in the way of reaching his full potential,&#8221; Brode says. &#8220;We work to maximize the relationships kids like Richard can have with their families.&#8221;<br />Besides Richard, Carter has a husband and another son, Vince, 2, to care for. Still, she is aggressively working to ensure Richard has the best chance possible. Besides working with Solomon, he is also enrolled in speech, occupation and music therapy programs. Carter, founding of the Boxing Autism Club, a support group in Livingston County and the executive director of the Michigan Autism Partnership, has become an autism activist. She works for change, pointing out that insurance companies in the state of Michigan don&#8217;t cover these recommended therapies for autistic children.<br />She also offers reassurance for other parents of autistic children.<br />&#8220;If you really want to learn, have the desire to do this and your heart is in the right place, you are going to be able to do this.&#8221;</p>

Project is designed to bring an intense program to children on the autism spectrum at a fraction of the cost of tradional ABA or Floortime.  The PLAY Project can also augment an existing ABA or tradional school program. <br />I will be providing PLAY Project services beginning the end of April 2010. 			<content:encoded><![CDATA[<p>A portion of Floortime is usually considered to be &#8220;follow their lead&#8221;.  This is often interpreted as doing what the child does and what the child wants to do.  I believe you should follow or acknowledge their intent.  The idea a child has cannot always be played out.  The child cannot have or do everything he/she wants.  BUT, we can acknowledge that we understand the intent and the idea.  I think it is critical in nurturing a relationship to let the child know that you do understand what he/she wants and you realize that there are emotions surrounding the wants. 			<content:encoded><![CDATA[<p>I received the lease for my new office today!!!  I am so excited about my new private practice!  I will be sharing an office space with 5 other professionals.  They are psychologists, a psychiatrist and a family counselor.  We share a waiting room, conference room, kitchen, file room, etc.  I will have a private office/therapy room with a storage closet. <br />I am dedicating this entry (and probably a few in the future!) to the steps I am going through to get my practice up and running.  I have had trouble finding this information on the internet so maybe my experience will help someone in the future.<br />The space &#8211; I sent out a couple of emails to find my office space.  I had quite a few choices.  I knew I wanted something fairly centrally located or at least easy to get to and a I wanted a quiet/private therapy room.  I think word of mouth is the best way to find office space but I did also see a lot of listings on Craigslist.<br />Billing -the next HUGE task is figuring out the billing issues.  The people I will be sharing an office with use Therapist Helper software so I am looking into it.  I have downloaded a demo and it is going okay so far.  I am taking an online class next week.  There are many options out there but I wanted something built for therapists.<br />Insurance &#8211; I don&#8217;t know much about this yet!  I am in the process of talking with Blue Cross Blue Shield.  I am a PAR with them but I want to be in network.  I am also planning to talk with United Healthcare about becoming a provider.<br />Tests &#8211; I have been looking daily for used assessment tools &#8211; these are very pricey so I have been trying to get a small collection through ebay, alibris, half.com, and amazon.  I primarily use tests for insurance purposes only.  I use professional observation and language samples as my true assessments so I do not want to spend a lot of money on tests I won&#8217;t use.<br />iTouch!!!!  &#8211; I had to get an iTouch b/c there is a new app for the iTouch that will make it an augmentative communication device.  This has been one of my most exciting purchases!<br />Business Cards/Promotional Materials &#8211; I ordered everything through Vista Print.  Nice and cheap!!!!! They also have many, many designs and choices of items.   I also plan to print a one page flyer about my qualifications to take to schools and physicians. <br />I already have a lot of potential patients!!!  I cannot wait to get started!<br />Betsy</p>

			<content:encoded><![CDATA[<p>I am well on my way to building my new business!  Before I had my own kids, I had a very successful private practice.  I loved it.  Loved being my own boss.  Loved treating kids the way I wanted to treat them and working with families the way I wanted to work with them.  In 2000, when Henry was  2, we moved to Houston for a job for my husband and I left my business behind.  I missed it but needed to focus on Henry and his medical issues. <br />Nine years later, I am about to start up again!  This time I am planning to get an office space and I am very excited about that.   I made the decision about 2 weeks ago and I have a firm, well thought out plan in place and am looking into some insurance billing.  I also should be able to take credit cards immediately.  <br />I will continue providing therapy using the DIR/Floortime philosophy.  I will continue my tutoring with my wonderful mentor, Michele Ricamato in Chicago and I am looking for a mental health tutor as well.  I am so excited to have the opportunity to bring Floortime therapy to more children and families!<br />I also plan to provide oral motor and feeding therapy utilizing Floortime.  The basis for the therapy with be the relationship.  I have loved doing feeding therapy over the 17 years of my career as a Speech Pathologist and Floortime adds a new, deeper dimension to my work in feeding.<br />As I order new assessment tools and other pieces of equipment, I get more and more excited about this new chapter in my life!!!! 			<content:encoded><![CDATA[<p>As I build my blog, I am forced to think about what is important to me professionally.  I have been a <span class="blsp-spelling-error" id="SPELLING_ERROR_0">SLP</span> for 17 years.  Over these years, I have worked with many children and families and learned so much from all of them.  The most valuable experiences have come from the interactions I have had from families.  Early in my career, I worked in <span class="blsp-spelling-error" id="SPELLING_ERROR_1">ECI</span> and I learned the value of including the family in the treatment.  I have never been able to get away from that model.  When I shifted to <span class="blsp-spelling-error" id="SPELLING_ERROR_2">Floortime</span>/DIR almost 2 years ago, it was a perfect move.  I love including the family &#8211; in fact, the more, the merrier.  I love being able to get to know both the immediate family and the extended family.  It takes a village to raise a child and that village HAS to know how to interact and successfully communicate with the child!<br />The other <span class="blsp-spelling-corrected" id="SPELLING_ERROR_3">crucial</span> piece is the relationship between the child and the therapist.  Without a great relationship, therapy is very difficult if not impossible.  When I get a new client, I have always started therapy by getting to know them.  This also is a perfect fit with <span class="blsp-spelling-error" id="SPELLING_ERROR_4">Floortime</span>/DIR.  The relationship carries the therapy.  Engagement is key.  If the child is not regulated (calm), engaged and attending, you will not get anywhere with therapy.  You must have a relationship to maintain and build engagement.</p>
